You are authorized to enter into this Agreement on behalf of the company or organization named above.

You are providing your electronic signature voluntarily, and it will have the same legal force and effect as a handwritten signature on a paper document.

The date, time, IP address, and a copy of this signed Agreement will be recorded and stored as part of the execution record.

This Agreement becomes effective immediately upon execution by both parties.
